Initial Physical Therapy Evaluation
For families who have a specific concern or have been referred/recommended to physical therapy.
An initial evaluation is a comprehensive assessment performed by a skilled pediatric physical therapist to better understand your child's movement, strengths, and areas where they may benefit from additional support. Your visit includes:
- assessment of your child's current physical and gross motor skills
- identification of strengths and areas of concern
- discussion of your child's movement and development
- personalized activities and exercises to work on between sessions
- recommendations for continued care, when appropriate
This option is intended for families anticipating ongoing physical therapy services.

Initial Visit
Child Development Consultation
You would like to receive personalized advice to promote continued age-appropriate development.
A child development consultation is designed for parents who don't have a specific concern, but would like guidance from a skilled pediatric physical therapist on how their child is progressing and what they can work on next. During your consultation, we'll:
- take a quick look at your child's current developmental skills by observing movement and play
- trial more advanced skills when appropriate
- discuss what skills you can encourage at home
- provide tangible activities and ideas to support your child's development
This option is intended for a one-time consultation unless additional support is recommended.

Follow-up Therapy Visits
After an initial evaluation, we can create a visit schedule based on your child's individual needs and goals.
30-minute weekly visits
$75/visit
For younger infants who may benefit from shorter, more frequent sessions, due to endurance, routines, and rapid growth.
60-minute weekly visits
$110/visit
Ideal for children who benefit from consistent weekly intervention and caregiver education.
60-minute monthly visits
$125/visit
A great option for children who need occasional skilled guidance while continuing to practice skills at home.
MOTM Punch Card
Purchase a package of visits to use as needed for ongoing support and flexibility based on your child's needs.
$1000 for 10 visits
HSA & FSA Accepted
Minis on the Move is a self-pay pediatric physical therapy practice. Eligible services may be paid for using HSA or FSA funds. We provide detailed receipts for your records and reimbursement requests.
HSA/FSA eligibility is determined by your individual plan. Please contact your plan administrator to confirm coverage.

Superbills provided for potential out-of-network reimbursement.
Minis on the Move is a self-pay pediatric physical therapy practice. We do not bill insurance directly. For families with out-of-network benefits, we provide a detailed superbill that can be submitted to your insurance company for potential reimbursement. Please contact your insurance provider before your first visit to determine whether your plan includes out-of-network physical therapy benefits.
Insurance reimbursement is determined by your individual insurance plan, and reimbursement is not guaranteed.
Cancellation Policty
We understand that life with little ones can be unpredictable. To respect our therapists' time and keep appointments available for families who need them, cancellations made less than 24 hours before your scheduled appointment will be subject to a $50 cancellation fee. Thank you for helping us keep our schedule running smoothly.
